Pick up of ordered contact lenses is 2 to 3 days

If the patient prefers 2 pairs of graded contact lenses, an additional P100 will be paid directly to the merchant
Only one patient will be checked by the clinic's doctor for the release of prescription
If 2 patients will have their eyes checked, an additional P250 will be paid directly to the merchant for the doctor's fee
Instant pick up of contact lenses after eye check is only applicable if items are available in stock
Free lens case will be for storage of extra contact lenses and foil will be surrendered to White House Dental and Optical Clinic's cashier for the inventory of promo deal
Contact lenses will be fitted or inspected by the patient before leaving the clinic
Disposable colored contact lenses are good for 3 to 6 months
Due to the fragile, soft, and delicate characteristics of contact lenses, no warranty policy will be applied
Price is inclusive of taxes and service charge
Voucher is not valid in conjunction with Senior Citizen card and any discount card or promotion
Voucher is transferable and may be given as a gift to family and friends
